Pre-consult Questionnaires

This page is for clients that have booked a consultation and have been asked to complete a pre-consult questionnaire if it’s their pet’s first time visiting us. Please arrive 5 to 10 minutes before your consultation to allow time complete any additional paperwork.

If this is your first time visiting us, please take a photo of your pet’s living area or enclosure, as well as their diet. This is not always needed, but does often help, as some set ups and diets can be hard to explain.

If your concern about your pet is a behaviour that they are doing intermittently, then it is often helpful if you can take a short video of the behaviour on your phone. That way if they don’t do it during their visit we can still investigate it.
